Cases of Harm Reporting Form
Purpose
The purpose of this form is to report cases in which harm occurred from inappropriate use of genetic information or genetic test results. Collecting these cases will help determine the extent to which not having genetic counselors regulated in Michigan may be harmful to citizens of this State.
Types of Harm
Types of harm to report include but are not limited to physical, emotional, and financial resulting from failure to recognize genetic risk factors, misinterpretation of genetic test results, and/or failure to communicate relevant genetic history, risk factors, or test results by or to a referral source. Cases to report include individuals who have had unnecessary medical procedures, those who were not provided with appropriate intervention/surveillance for a medical problem, instances of nondisclosure of genetic risks, delayed or incorrect genetic diagnoses; and/or emotional and financial harms resulting from any of the above. This is not an exhaustive list, but serves as examples for cases to be reported.
